I mean, the bet was that she had to make Keisuke hate her so much, that he would kill her. She really had to make sure he hated her from the bottom of her heart. In Nemu's route he almost kills her but then stops right before he is about to strangle her, which means she wasn't even cruel enough in that scenario. And in the rescue game before they get out of the underground facility, he jumps in front of the shooting knives to save her, so maybe that made her realize she really needs to be as cruel as possible? He basically showed her that even after she treated him like a bitch for so long he still wanted to save her. So she had to make him hate her even more. Quote

She was too genuinely cruel to think she was acting. There's no real timeline given so the time between Keisuke getting captured by them and him finding Bakuya in the facility could be as long as the writers wanted. Who knows, maybe several months have passed by that point. Any length of time is possible, so you shouldn't assume that it was a short length. And yeah, she was really faking the sadism. There were signs of her true self at several points throughout the story, like when she tended to Keisuke's hand, and several flashes of remorse that she let slip. Quote
