In the visual novel world, it usually means a game that already comes with the translation patch installed so you wouldn't have to do it yourself. I.e Clannad pre-patched means Clannad already in English Quote

However, this depends on your technical savvy, and for a decent few releaes, there's really no issue with pre-patching. One of the problems is patch updates, where bugs and typos are fixed etc. These require updates to the pre-patched release on behalf of the distributor, which might not be done as it's a decent bit more of a pain than just applying a new patch. One example of the latter would be the no-longer-available-on-Fuwanovel release of Ikikoi, which had an annoying out-of-sync-voices bug until you donloaded a fix. Quote
